On top of addressing the medical issues...
⬜ Do I need to consult a service?
Call in the AM, and avoid end of day or ‘last minute’ consults
Some services will request a Dovetale order AND call: psychiatry, geriatrics (order as ‘gerontology consult’)
Some services that need a Dovetale order only, but call if urgent/complex: interventional radiology, diabetes educator, wound care, allied health, spiritual care, ICC, CDCBT (Capacity Building Team)
⬜ Are all medications ordered appropriately?
Can I switch parenteral meds to PO?
Renal/hepatic dose adjustments: pharmacist can help
Hypovolemia/poor oral intake: sick day meds (SADMANS), antihypertensives
Bleeding: pharm VTEp, antiplatelet/anticoagulant
Diarrhea: laxatives, metformin, potassium elixir, magnesium etc.
Confusion: assess psychotropics
⬜ Can I D/C Foley, cardiac monitoring or other lines/drains/tethers?
⬜ Have I addressed VTEp and PRNs? - analgesics, antiemetics, bowel meds, nicotine replacement
⬜ Does this patient need more psychoemotional support?
Consider recreational therapy, spiritual care or psychology for patients struggling with their illness experience; not all depressed/anxious patients need a psychiatry consult
⬜ Do I need to (re)address code status?
End of day tasks
⬜ Order tomorrow's labs if needed
⬜ Update handover text and PMHx
⬜ Update family/POA if patient is unable to
Forgot a patient's name and they're not on a team list? At the top banner, select Patient Lookup > Recent Patients
Method 1 - Patient Lists screen > select Write Handoff
Method 2 - enter a patient's chart > select Handoff in the top right corner
Dovetale’s search function is very useful.

SmartPhrases are used to enter pre-defined text, which can include patient-specific variables (vitals, results, meds, I/O, demographic info, etc).
In the note writer, type . (period) and begin typing a keyword to search a SmartPhrase.

Make a new note > RIGHT click inside the white space > SmartPhrases > Create New SmartPhrase, OR highlight existing text and select Create SmartPhrase from Selected Text
The name of the SmartPhrase will also be its keyword for use - e.g. if the name of your admission note template is 'ADMIT', type .ADMIT in the NoteWriter to use it
You can include other SmartPhrases in your custom SmartPhrase, by surrounding the SmartPhrase keyword with @ (for example, @ALLERGY@ instead of .allergy).
Use Windows’ Snipping Tool (Windows logo key + Shift + S) to make a screenshot, and simply paste into your note
View the note > select Route
Remember to route your admission/discharge note to the patient’s GP and relevant specialists
In the Routing pop-up, search the name of the recipient in the search bar. Click 1 PCP to add the patient’s family doctor. Click 4 Free Text to send to a custom fax number.
If the recipient is a Dovetale user, this will send the note to their Dovetale inbox.
If the recipient is outside SJHH (such as their family doctor), this will send a fax electronically.
